Fixed dentures are made of natural teeth on both sides or one side of the missing tooth space as abutments, and the retainers of fixed dentures are made on the abutment teeth, and connected with the artificial teeth as a whole, and then fixed on the abutment teeth by binders.
1. The number of missing teeth
Fixed dentures are most suitable for repairing 1~2 missing teeth, or missing teeth in more than two and are spaced missing, with abutments in the middle to increase support, the number of missing teeth and the missing area must be considered, and the ability of the abutments on both sides to withstand the jaw force. Failure to do so will result in the repair failing.
2. The location of the missing teeth
As long as the number of missing teeth is small at any position of the dentition and the abutment teeth meet the requirements, fixed denture restoration is applicable. If the second molars are free and missing, the opposite jaw is a removable denture, and the missing side can be restored with a single-ended fixed bridge using the first molars and the second premolars as abutments.
3. The condition of the abutment teeth
As a fixed denture abutment, the clinical crown should have a suitable height and normal morphology. If the clinical crown of the abutment is too short, an auxiliary retention device can be added, or the number of abutments can be increased. The root of the tooth should be thick and stable, supported by multiple teeth, and there should be no pathological loosening, and a viable pulp is the best. If there is a lesion in the pulp, it is necessary to undergo thorough endodontic treatment and observe asymptomatic for three months before it can be used as an abutment. Periodontal tissue should be healthy and disease-free, or alveolar bone should be resorpted horizontally and not exceed 1/3 of the root length. The axial position of the abutment tooth is required to be normal, without excessive tilt or torsional dislocation, and it does not affect the preparation of the co-positioning channel in the preparation process of the retainer.
4. The occlusion
The occlusion in the edentulous area should be basically normal.
5. Generally three months after tooth extraction
Fixed dentures are made after the alveolar ridge absorption is basically stable. The alveolar ridge should not be absorbed too much in the edentulous area, especially in the anterior area.
6. Suitable age for fixed denture restoration
It should be 18~60 years old. If the age is too young, the crown is short, the pulp cavity is large, the pulp angle is high, and the pulp is easily damaged during the preparation of the abutment teeth. If the age is too old, the periodontal tissue is significantly atrophied and the tooth is loose, and fixed dentures should not be used for restoration.
